We are seeking a Social Service Rep gathers information, responds to admission inquiries and coordinates with nursing staff on inpatient admissions and discharges as a part of an interdisciplinary team. Serves as an information resource to families regarding programs and available services.
In this position you will have the following responsibilities:
Assess need and actively network in the community to increase Benedictine's service base and identify services needed. Establish and maintain ongoing relationships with referring agencies and individuals.
Respond to admission inquiries and complete admission forms. Ensure that necessary authorizations, screening and funding are in place prior to the admission of any resident. Coordinates admission with DNS, Administrator, and other disciplines when appropriate.
Collect social history data and completes assessments on residents upon admission and as needed according to department guidelines
Actively participate in the development of discharge plans in coordination with the interdisciplinary team, families and outside agencies.
Develop a therapeutic relationship with families/guardians, facilitating communication with the interdisciplinary team. Communicate issues and follow-up to the appropriate person(s) and documents information in the medical record. Provide information, assistance and one-on-one support to residents and families
Participates in interdisciplinary team care conferences by providing placement goals and objectives for residents, and by acting as a resource person to help coordinate and establish a resident care plan. Work closely with Resident Care Managers, Chaplains, Life Enrichment Coordinator and Charge Nurse in identifying and meeting on-going social and emotional needs of residents
Serve as a resident advocate, especially in relation to the 'Resident Bill of Rights.' Communicate special needs and concerns to families, and conversely brings concerns of family members and residents to staff
Actively participate in department staff meetings and organizational training sessions.
Required qualifications for this position includes:
Associate's Degree in Social Services or related field or 2 years of social services work experience.
1 year Social services work experience in a long term care or health care facility.
In Compliance with the Department of Human Services (DHS), a criminal background check is required for this position.
