We are seeking a Physical Therapist - Home Health Pbnc Salem Branch that will manage and provide skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician's plan of treatment, taking into account individual patients special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential.
In this position you will have the following responsibilities:
At the physician's direction, evaluate and assess the patient in the home
Plan and administer medically prescribed physical therapy treatment for patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body part
Demonstrate ability to complete a comprehensive physical assessment and identify need for referral to additional appropriate clinical disciplines
Perform patient tests, measurements, and evaluations such as range-of motion and manual muscle tests, gain and functional analysis, and body-part measurements
Direct and aid the patient in active and passive exercise, reeducation, gait training and functional training and activities of daily living including transfer activities and prosthetic training
Record and evaluate tests, measurements, and evaluations to aid in establishing or revising specifics of treatment programs
Administer therapeutic exercises to improve or maintain muscle function
Instruct, motivate, and assist patients in exercises, such as isometric, passive, and progressive-resistive exercises; and in functional activities, using weights, pulleys, exercise machines, steps, and assistive and supportive devices such as wheelchairs, walkers, crutches, canes, parallel bars, prosthetic and orthotic devices
Administer treatments involving applications of physical agents such as moist packs (heat or cold), massage, electrical stimulation, TENS, oximetry, and ultrasound machines, under a physician's order
Record patient treatments, response, and progress using a point of care system to meet department standards, State and Federal regulations in a timely and accurate manner
Maintain accurate, thorough and timely documentation in accordance with department policy and procedures (e.g., evaluations, progress notes, discharge notes/summaries, charges, functional outcomes).
Confer with physician and other health practitioners to obtain additional patient information, suggest revision in treatment programs, and integrate physical therapy with other aspects of patient health care
Report patient's reaction to treatment and any changes in condition to the physician
Provide written and verbal instructions to the patient/caregiver to assist the patient in resuming life in the community or in an established safe environment. Identifies specialized needs requiring specialized follow-up after discharge and making the necessary referrals
Instruct patient and family in physical therapy procedures to be continued at home
Direct and Supervise physical therapy assistants and aides
Provide quality care in an efficient, cost effective and caring manner
Demonstrate flexibility to meet service area/department's demands while maintaining department productivity standards as defined for Home Health by visits per day. (e.g., adjusts daily schedule to complete workload, prioritizes tasks, initiates a positive approach to problem solving)
Demonstrate advancement of professional physical therapy practice through continuing education
Participate in one activity annually which maintains/enhances professional expertise and clinical skills (e.g. continuing education courses, workshops, inservice presentations, involvement in professional special interest groups, self study, literature review, videos, educational forums, etc.)
Attend team/staff meetings and departmental continuing education in-services
Maintain a positive, professional presentation at all times, (e.g. appropriate appearance as per department policy, composure, sound judgment, positive coping behaviors, and adjustment to performance as directed by Supervisor and Manager.)
Participate in the Quality Assurance process as requested
Establish and promote positive working relationships both within the system and throughout the community
Participate as a preceptor in the orientation of new staff and/or students as assigned by the Clinical Supervisor
Required qualifications for this position includes:
Bachelor's Degree -OR- Master's Degree in Physical Therapy; from an accredited institution
Physical Therapy licensure through the State of Oregon
Current Basic Life Support (BLS) for Provider Certification or ability to obtain in sixty (60) days
Renewal of BLS card every two (2) years
Driving record consistent with Safe Driving Policy 330 Incident Point Matrix at time of hire
One (1) year of clinical experience in a healthcare setting, preferably home health, or active enrollment in the Providence Benedictine postgraduate preceptor program
