We are seeking a Physical Therapy Assistant - Home Health Services that will provide skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities, taking into account individual patients' special physical or age-related needs. Requires skills in patient/client treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with individual's functional level and potential.
In this position you will have the following responsibilities:
As established by the physical therapist, treat patients to relieve pain, develop and restore function and maintain maximum performance using physical means, such as exercise, massage, heat, cold, ultrasound and electrical stimulation
In accordance with the plan of treatment, direct and aid the patient in active and passive exercise, re-education, gait training and functional training and activities of daily living including transfer and prosthetic training
Report observation of patient's/client's reaction to treatment to the physical therapist
Manage a caseload as assigned by the physical therapist
Manage equipment safety and adapts, fits and instructs in the care and use of equipment to the patient/client
Teach/instruct patient/client and caregivers to assist patient/client in achieving treatment goals
Confer with physical therapists, physicians, families, and other caregivers regarding treatment plans, expected results and goals
Record clinical, progress notes and itinerary records to meet department standards, State and Federal regulations in a timely and accurate manner
Conference with other Home Health staff on each patient to maximize optimal patient outcomes
Terminate patient/client treatment, upon approval of physical therapist, based upon condition/diagnosis and functional performance outcome expectations
Provide quality care in an efficient, cost-effective and caring manner
Participate in the Continuous Quality Assurance process as defined in the department CQI policy
Establish annual individualized goals for own professional development
Demonstrate advancement of professional physical therapy practice through continuing education
Demonstrate advancement of professional physical therapy practice through community professional organizations, Home Health activities, or continuing education
Establish and promote positive working relations both within the system and throughout the community
Meet or exceed department productivity standards as defined for Home Health by visits per day
Required qualifications for this position includes:
Associate's Degree in Physical Therapy
Physical therapy Assistant licensure through the State of Oregon
Current Basic Life Support (BLS) for Provider Certification or the ability to obtain in 60 days
BLS card to be renewed every two (2) years
Valid driver's license
Driving record consistent with Safe Driving Policy 330 Incident Point Matrix at time of hire
