Providence RN Case Manager - Benedictine Assisted Living in Mount Angel, Oregon
Providence is calling a RN Case Manager to its Benedictine (Orchard House) Assisted Living Facility in Mount Angel, OR. This is a full-time, day shift position. You will work Monday-Friday with a shift start time of 8am. You will also be scheduled to work every other Saturday.
We are seeking a RN Case Manager that will assume primary responsibility for the Health Care Services for residents of Orchard House. Participates in service planning, as well as renewal and maintenance of these plans. Recommends residents goals and assists in their implementation. Works with the Housing Director to oversee, and coordinate the work of service partners. Provides clinical services to residents, implements/maintains delegation process of appropriate cares, and assists with education of care giving staff.
In this position you will have the following responsibilities:
Together with the Housing Director, resident, resident's family, and caregivers, plans, analyzes, and evaluates the needs of the resident to formulate the service plan of care
Participate in on-going service plan meetings
Participate in development, implementation, review, and necessary modifications to policies and procedures for resident care services and medication management
Perform on-going staff teaching, delegation of special nursing tasks and medication administration in accordance with the Oregon State Board of Nursing Administrative Rules, Chapter 851, Division 047
In conjunction with the Housing Director will oversee and review medication administration and Health Services:
Assure accurate transcription of physician orders into treatment/medication record
Review resident service records, treatment, and medication books to assure quality of care
Verify physician orders on move-in and continuously thereafter for information and initiates appropriate action/follow-up
Supervise ordering of medications from pharmacy
Document a nursing plan for the resident's service plan including delegation procedures, frequency of nurse follow-up visits, and signature and license number of the RN doing the delegating
Attend in-services/meetings and other programs, has 20 hours of documented Division-approved continuing education credits each year
Participate in QI activities
Maintain good rapport by interacting in conflict or stressful situations in a manner that facilitates constructive resolution
Provide ongoing health services to residents as needed, and coordinates health services with outside service providers such as hospice, home health, physicians' offices etc. when necessary
Communicate effectively and manages conflict in a positive manner
Maintain R. N. to staff communications book with current updates of residents changing health needs
Perform specific nursing tasks based upon facility policy
Examine and administers first aid to residents and personnel within scope of practice
Complete incident reports and notifies physician as appropriate
Work with the Housing Director to screen all applicants prior to move-in to ensure appropriate placement
Required qualifications for this position includes:
Current Oregon RN license through the Oregon State Board of Nursing
Cardiopulmonary resuscitation (CPR) Certified
In Compliance with the Department of Human Services (DHS), a criminal background check is required for this position
Preferred qualifications for this position include:
Three (3) years Experience in home health or geriatric nursing
Ability to work independently
