We are seeking a RN, Team Lead who will be under the direction of the Manager and on physician orders, is accountable for indirect quality care of patients according to their individualized needs. Responsibilities includes triaging calls, clarifying physician orders, coordinating patient care activities with other healthcare members; assisting with the oversight of clinical staff and scheduling; acting as a preceptor for new staff, and participate in after-hours call.
In this position you will have the following responsibilities:
Triage phone calls from patients, physician offices and caregivers. Provides instructions, direction of care and communicates patient needs appropriately.
Respond to and clarify physician's orders and enters them into the patient's EHR. Communicates orders to Home Health staff and Scheduling as needed. May assign and/or provided written instruction for Home Health Aide and LPN plans of care.
Assist the Clinical Supervisor with oversight of clinical staff, scheduling, running weekly reports for Team Conference, directing Team Conference and attending department meetings as requested.
Participate in the agency Quality Assurance process for chart audits and patient certification order processing.
Maintain current knowledge base of payers (insurance) and department reimbursement requirements and processes. Maintains current knowledge of applicable Home Health regulations. Act as a resource to staff regarding requirements, processes, and regulations.
Act as a preceptor in the orientation of the new staff. Co-facilitates orientation completion of new caregivers with supervisor to ensure documentation and competencies are completed.
Participate in after-hours call as contact for caregiver for triaging patient or clinician needs and concerns.
Required qualifications this position include:
Education to meet certification, license or registration requirement.
Current Oregon RN license.
Current BLS (Basic Life Support for Health Care Provider) or ACLS (Advanced Cardiac Life Support) required within 30 days of hire.
2 years medical/surgical experience in an acute care setting.
2 years recent Home Health experience.
In compliance with company policy and state regulations, a Department of Human Services (DHS) criminal background check is required for this position. Fingerprinting may be required also.
