We are seeking a Wound Ostomy Nurse Certified, Non-Contract - Home Health Pbnc Salem Branch that will be accountable for direct and indirect quality care for patients according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. The CWON will function as an educator and consultant to the Home Health Services staff, patients, and in the community as needed. The CWON will coordinate patient care activities with other members of the health care team in a collegial relationship.
In this position you will have the following responsibilities:
Utilize the nursing process, assesses patient status, intervenes appropriately, and evaluates patient response
Establish a formal written plan of care for referred patients, documents specific nursing orders, and develop goals specifying expected outcomes of intervention
Assess health knowledge, learning abilities and educational needs of the wound care and ostomy patients and families and develops appropriate plans for teaching
Utilize all available resources and consults as necessary with members of the health care team in planning and developing individualized teaching and discharge plans
Complete all necessary documentation according to regulatory standards
Function as a knowledgeable resource person to the nursing staff in areas of expertise, related to ostomy care, prevention and treatment of wounds of all types, and management of the incontinent patient
Serve as a resource person to clinics, physician's offices, and businesses in the areas of expertise
Review literature, methodology, outcomes, and implications of nursing research, and disseminates this information to clinical and medical staff
Identify staff learning needs, provide the necessary staff education in collaboration with RN Educator
Participate as preceptor in the orientation of new staff and/or students as assigned by the supervisor
Work closely with Home Health Services supply management, medical equipment vendors, and outside vendors as needed to coordinate availability of needed wound care products, and to improve quality of service to patients
Participate in product evaluations of new equipment and supplies related to areas of expertise
Actively participate in Home Health Services quality management programs and department meetings
Continue professional development and demonstrates advancement of professional nursing practice through both internal and external continuing education opportunities, membership in professional organizations, participation on Home Health Services committees
Required qualifications for this position includes:
Current State of Oregon Nursing license
Certification as a CWON
Current Cardiopulmonary resuscitation (CPR) certification for Health Care Providers
Current driver's license
Two (2) years CWON experience
Preferred qualifications for this position include:
- One (1) year CWON experience in a Home Health setting
